These are the documents you’ll need to register your child:
Parent/s Information
- Photo identification
- Pay stubs – 6 recent bi-weekly or 12 recent weekly
- Income verification form (to be completed by your employer before registering your child)
- School or training schedule, if applicable
- Proof of address in the form of one of the items below:
- Utility bill
- Lease
- Letter from your landlord
- Letter from a governmental agency confirming your address
- Parent Affidavit of Residency, if you are sharing a household
Child’s Information
- Birth certificate for all children under 18 years old
- Immunization card
- Medical card
- Social security card
